For training purposes, telephone calls may be recorded or monitored.
Information we require to provide policyholder details
If you are advising an Equitable Life policyholder, we will require the following:
- A request from you on company headed paper with details of your requirements.
- A letter of authority which needs to include the policyholder or grantee(s):
o Full Name
o Full Address & postcode
o Policy number (if known)
o National Insurance number
o Date of birth
- The letter of authority must be signed and dated by the policyholder or grantee(s). A letter of authority will expire in the event that no contact is made by the authorised persons during any given two year period.
